The decision to adopt internationally is the start of a new phase in your life. For many families, adopting internationally is the most exciting and important event in their life resulting in the contentment of having a child and completing their dreams of a family. While this route can take you to success it is not so simple and has quite a few risks associated with it. One of the issues of international adoption is to understand why children are available for adoption. The chief reason for availability is low income. With poor income comes health risk. It is certainly not implied that all children are unhealthy, but to identify that a primary issue to plan for in your international adoption is how to assess and screen for health issues. While educating yourself is the first step in health assessment, selecting an physician of international repute for check up is the most important step.

Since health risk is the most important issue for any international adoption, the ability to find out the medical info about the child and processing this info correctly is vital. Considering a typical Russian adoption as our model for discussion, a family would either be sent medical report prior to traveling or would be allowed to review the referral child's medical records during the 1st trip to meet the child. The reports are written in Russian and need translation. Translation of medical records can be intricate and can possibly contain mistakes that an inexperienced person may not recognize. Moreover, the Russian healthcare system is not the same as the US system. An example is in diagnosing. Those accustomed with Russian medicals realize that the Russian medical professionals are generally more severe and critical medically and they tend to make diagnosis without substantiation. In contrast, a US physician will generally only make a diagnosis when there is substantiating evidence to back the verdict.

Early in our adoption experience at Adoption Options, Inc. we would bring older kids to the US via our Cherry Orchard program as a host program. These children were from 5 years of age to 12 years of age. They would stay with host families and be given the chance to meet potential adoptive parents. As part of this program we would plan for the children to be evaluated by physicians. In one specific program, we set up with the director of pediatrics of a university teaching hospital to assess the children using his residents. The director had vast international experience. The evaluations were made up of reviewing the medical records provided by the Russian authorities and undertaking a cursory physical check of each of the children. At the end of the evaluations this physician invited us to attend a teaching conference with the evaluating residents where he had each resident present their findings. The prominent outcome was that after each presentation he informed each of the residents as to the mistakes of their conclusions based upon his international exposure. The lesson to learn was that families cannot rely on a simple assessment but one performed by a physician that had international experience.

Luckily for families who wish to adopt children there are a number of physicians and clinics in the United States that are competent and offer helpful evaluations. You can locate these services by asking your local adoption agency, your placement agency, your local region specific support group, or by searching over the internet. The services provided vary greatly, so talking with people who have already availed their services can be very helpful. If the country with which you are doing your adoption allows you may be able to arrange for an independent adoption doctor to appraise on site during your first trip.

Screening for health issues is totally essential with international adoptions. Such assessments add to the cost but this is not something anyone should ever try to do away with. Adopting children internationally is a viable way to complete your family but it does require being prepared.
