This is commonly the case for more costly test, like CT and MRI scans, surgery or visits to specialists.

For some kinds of covered medical treatment, there are limitations on the number of treatment that are covered annually. This frequently includes mental health treatment and physical, occupational and speech therapy.

Remember that your insurance company, not your doctor, make decisions about what will be paid for and what will not.
